•Required documents: Proof of identity (e.g., ID card)
Additional documents such as a power of attorney may be required for applications submitted by a representative (details will be provided via WhatsApp).

•Refund processing schedule: Refunds will be processed and deposited sequentially starting in early February, in the order applications are received.

2. Medical Record Retention and Copy Issuance

■ In-clinic issuance period: Even after operations end, copies of medical records can be issued directly at the clinic until Friday, February 13, 2026.

■ Required documents: For personal requests: Identification (additional documents may be requested depending on circumstances)

-Record storage and issuance after February 13: After February 13, medical records will be preserved electronically in accordance with medical law.

-How to obtain records after closure: Records may be issued through the relevant public health center after identity verification

-Identification is required for issuance, and additional documents such as a power of attorney may be required for representative requests.
